Outreach is a flexible option that can offer you some extra support. It can be simple things that are beneficial in the long run like social contact, having the opportunity to get out and about, or continuing a hobby or interest.
Typically we use a person’s home to develop a care plan that suits the situation with the care provider, & also meets the person’s needs and budget.
Everyone’s needs are different but once you have established the outreach care system in place, we tailor our service to any circumstance at any time.
The individual needing care or the family member is responsible for deciding how the needs will be met.
We Support people to:

- Transport to activities & appointments: we will organise transport to make it easier for you to enjoy social activities and make your medical appointments.
- Stay active & meet people: we can help you to remain active & socialise with existing & new friends.
- Get practical help around your home: our outreach workers can connect you to local handyman.
- Get housing & benefits information: we can help you to consider where to get professional advice about housing issues & we can support you to understand what benefits you are entitled to claim.
- Get access to health services: we can help you & support you to make, & attend your medical appointments & understand health care options that you have.
- regular companionship: our special skill staff can provide good company and interesting conversation through weekly visits.
